2020-2021 Planning Survey

The purpose of this survey is to determine how many families would be interested or would be able to attend GRACE Co-Op for the 2021-22 school year.  After we have an idea of the number of families/students, we will be able to look at class options and begin planning. We do not have exact guidelines of what changes may be needed to resume co-op, but we will no doubt need all families present for the day and as the saying goes, an "all hands on deck" type of commitment plan. 

QUESTION 1: Will your family attend co-op for the 2021-22 school year?    If YES, please continue with the survey. If NO,you may click the submit button at the bottom of the page (some of our families will be moving in different directions for next year and although we understand, we will definitely miss them).  Note: If your family will be taking this year off, but would like to return in the future, please indicate in the Notes section and your information will be retained through our website.

Would your family be interested in attending a full day only or 1/2 day ?

QUESTION 3: In the area below please list any options for your family to be willing & able to attend co-op. 

Ex. masks, prefer not to wear masks, social distancing, large classrooms, etc.

Would you be willing to teach? If so, what classes or age group do you prefer?

QUESTION 5: 

Which classes or age groups would you prefer to assist with during the day? 

QUESTION 6:

Please list your children (including ages & grade ) that will be attending co-op for the 2021-2022 school year.

QUESTION 7: 

If you have an elementary aged student (K-5th grade), what specific classes or types of classes would you like to be offered at co-op? 

QUESTION 8: 

If you have a Middle School or High School student, what specific classes or types of classes would you prefer / need to be offered at co-op?

QUESTION 9:  Since our classes may need to be setup differently than previous years, do you have class/ activity recommendations for our students this year?  Possibly an activity that your family has enjoyed during the pandemic. Please also list age range. 

Examples: outdoors, online classes,  enrichment classes, science labs to correspond to science curriculums at home, park days, PE games
